
When you are standing in someone’s driveway looking at a high-mile used diesel, the seller is motivated and you want to believe the truck is what he says it is. That’s exactly when you need someone who has no stake in the outcome and knows what to look for. A good independent diesel mechanic has seen every way a high-mile truck can look better than it is. They know where the problems hide and they know how to find them before you sign anything.
We asked a diesel mechanic with years of roadside and shop experience to walk us through what he actually does when someone calls him out for a pre-purchase inspection. Not the textbook version. What he does.
He grabbed his flashlight and a pair of gloves and started talking.
A good diesel mechanic doesn’t pop the hood first. He starts the truck cold, walks straight to the back and watches the exhaust for ninety seconds. Color tells him more in that first minute than most people learn in an hour of poking around under the hood.
White smoke on a cold start that clears quickly is normal. White smoke that keeps going after the engine warms up is coolant getting into the combustion chamber, which means head gasket, cracked head or something worse. Blue smoke is oil burning. Black smoke at idle on a diesel that isn’t under load is a fueling problem, a sticking injector or a boost issue. Light grey that disappears fast, that’s fine.
Most people skip this step because they are eager to get under the hood. That eagerness costs them. The exhaust tells you whether to keep looking or walk away in the first two minutes.

Pull the dipstick to check the oil’s condition and level, wipe it, look at the oil on the rag. Color you expect to be dark on a used diesel, that’s not the concern. You are looking for texture and smell. Gritty oil means metal particles, which means something internal is wearing faster than it should. Oil that smells like fuel means the injectors are leaking down into the crankcase. Either one is a reason to stop the inspection and have a harder conversation about price.
Pull the oil fill cap and look inside with a flashlight. Sludge buildup tells you the truck went too long between changes, repeatedly. A little residue is normal. Heavy deposits on the cap or down in the valve cover mean the oil was neglected and neglected oil in a diesel at high mileage is the kind of problem that doesn’t announce itself until it’s expensive.
Check the oil level too. Low oil on a truck someone is trying to sell you is not an accident.
Pull the coolant reservoir cap and look at the underside. Pink or orange residue, that oily film you sometimes find there, means oil is mixing with the coolant. That’s a head gasket or an oil cooler failure and neither one is a small job. On a 6.7 Cummins the oil cooler is a known weak point on trucks from 2010 to 2014. Every platform has its failure points and a mechanic who runs the same corridor regularly knows all of them because he’s fixed all of them.
Most manufacturers spec a specific coolant type and if someone has been topping it off with the wrong kind for years the chemistry goes bad and starts corroding the cooling system from the inside. The coolant should be the right color and it should be clean. Murky coolant in a diesel with 200,000 miles is a red flag worth pressing on. Ask when the coolant was last flushed and replaced. If the seller doesn’t know, that’s an answer.
Don’t pull the turbo. Listen to it. With the engine warm rev it lightly and listen for a whine that shouldn’t be there, a wobble in the spool, anything uneven. Then use a flashlight and look through the intake pipe at the compressor wheel. Nicks or missing pieces on the wheel mean something went through the intake. Scoring on the housing means the bearings are going. Either way the turbo is on its way out and a replacement on most heavy diesel platforms runs $1,500 and up, more if it’s on a platform with difficult access.
Check shaft play by hand. A small amount of radial movement is normal, excessive movement is not. Most buyers don’t know how much is normal and that’s fine, which is exactly why you bring a mechanic. This is the kind of thing that takes a few hundred inspections to develop a feel for and there’s no shortcut to it.
Check the transmission fluid color and smell. Dark fluid with a burnt smell means the transmission has been overheated, probably from towing more than it should have or from fluid that was never changed. On the 68RFE automatic behind the Cummins this is a common find on high-mile pickups and it matters because a rebuild runs $3,500 to $4,500. On a heavy truck the numbers get bigger depending on the platform and who does the work.
If the truck has an accessible drain plug drop a small amount and look at what comes out. Metal flakes in the fluid tell the same story as metal in the oil. Something is wearing and wearing fast. The transmission is where high-mileage trucks tend to hide their worst secrets because the engine can look fine while the transmission is already failing quietly. A lot of sellers don’t even know the transmission is going. Some do.
Get under the truck. Bring a creeper or a mat and get under it properly, not a quick glance from the side. Do this on every inspection regardless of where the truck is from, but take longer on trucks that have spent time in states where they salt the roads. A diesel engine can be rebuilt. A rotted frame cannot.
Look at the welds, the crossmembers and the areas around the spring hangers and control arm mounts. Check the brake lines because steel brake lines on a high-mile truck can rust from the inside out and fail without warning. Check the fuel tank straps because those rust through quietly and a falling fuel tank is not a problem anyone wants to deal with on the highway.
Suspension components tell you how the truck was used. Bent parts, torn bushings or evidence of a hard impact that was never repaired show up down there if you are looking. A truck that’s been in a hard accident and had it patched over rather than properly repaired will show you the truth from underneath even when the bodywork looks clean.
Drive it yourself, or have your mechanic drive it. Not around the block. Take it out on the highway, get it up to speed and put some load on the drivetrain. Listen for vibration, feel for pull, watch the gauges and pay attention to how the transmission shifts under real throttle. A transmission that shifts fine at light throttle can behave very differently when you ask something of it.
Do a hard stop from about 45 miles an hour in a clear spot to feel the brakes and check for pull. When you get back let the truck sit at idle for five minutes, watch the temperature gauge and check underneath for fresh leaks. If the cooling system has a slow leak it will show itself after the engine has been running and the system is pressurized.
Most buyers drive timidly on a test drive because they’re anxious about the purchase. Don’t. Drive it like it’s yours, because if you buy it, it will be.

A thorough pre-purchase inspection on a high-mile diesel takes an hour to ninety minutes done properly. It covers the exhaust, the fluids, the turbo, the frame, the drivetrain and the test drive. At the end of it you should have a clear picture of what the truck is, what it needs in the next six months and what it’s likely to need in the next two years.
About one in four high-mile used diesels has at least one issue serious enough to change the negotiation. About one in ten isn’t worth buying at any price the seller is likely to accept. A pre-purchase inspection typically runs $100 to $150 at an independent shop or through a mobile mechanic. It is the cheapest part of buying a used truck and the part most people skip.
Don’t skip inspection. The truck will tell you everything you need to know if you give it the chance.
